If you have an injury, joint or muscle pain, ask your primary care provider for a referral to the Fort HealthCare Therapy & Sport Center nearest you. Or, for many common problems, you can self-refer to our clinic. For many athletic training and fitness conditioning programs, a physician referral is not required. Many health insurance companies cover the cost of athletic training and physical therapy services. You should contact your insurance provider to inquire about coverage before your appointment. If your insurance does not cover the services you need, you can still receive care – We’ll work with you to develop the best care and payment plan that will work for you.

What to expect on your first day of therapy

On your first day of therapy, plan to arrive 10-15 minutes early at one of our convenient locations in Fort Atkinson, Lake Mills, or Whitewater. You will be greeted by a friendly face to help you go through the registration process, including reviewing insurance information (be sure to bring your current insurance card and a photo ID) and personal information such as workplace and contact information.

We’ll then introduce you to your primary therapist who will complete an evaluation that takes about 60 minutes to determine what your concerns and needs are related to your desired outcome. Your therapist will collaborate with you to develop an individualized treatment plan which will include goals based on the evaluation and discussion with you.

Please dress comfortably for this first therapy visit. Be sure to wear comfortable shoes that are also appropriate for the type of visit you’re having – like athletic shoes with good support for most activities, or running shoes for a specific running visit, etc.
